ePathway, the electronic Care Pathway initiative

Two applications contribute to the overall electronic care pathway solution. The first (Nimbus Pathway) supports the development, implementation, monitoring and evaluation of the Care Pathway Journey (process map). The second (Formic Pathway) supports all aspects of the Care Pathway Document. Venture Training & Consulting offers training, facilitation and project management support for the initiative.

Both the Formic and Nimbus software applications are designed to compliment and add value to the local IT systems that hold the individual patient/client data, and as such are compatible with most other systems. In the UK their implementation assists with local uptake and adoption of the work being undertaken with regard to the Connecting for Health/National Programme for IT (CfH/NPfIT). Both applications are robust, have been around for many years and have a proven track record in their respective fields.

At the bottom of this page you will find a selection of documents for your information, covering:

    SE London User Network

I supervise 3 Clinical Governance Facilitators who work part time on ICP development at Queen Elizabeth Hospital in Woolwich, southeast London. We each have our own caseload of ICPs to develop and maintain. There are 43 ICPs in our programme and out of these, 14 in use in the Trust and PCT.

I would like to network with other Trusts in our region (and across the UK) and share our experiences by meeting up occasionally or by telephone/ email. I have a particular interest at the moment with learning how others are involving patients and carers with ICP development.

If you are interested, please contact Jane Mccombe, Queen Elizabeth Hospital NHS Trust by email jane.mccombe@nhs.net
